Service Information

• To provide multi-disciplinary professional outreach service to residents of private residential care homes for persons with disabilities, including rehabilitative therapy, psychotherapy and counselling, and social and recreational activities, so as to address their needs and enhance their quality of life.

• To provide professional advice, training and support to family members/carers of residents as well as staff of private residential care homes for persons with disabilities so as to enhance their ability for caregiving.

• To strengthen these residents’ connection with the community so as to promote their integration into the community.

• Residents of private residential care homes for persons with disabilities in Yuen Long District and Tuen Mun District (including physically disabled persons, intellectually disabled persons and ex-mentally ill persons)

• Staff of private residential care homes for persons with disabilities

• Family members or carers of the above-mentioned residents

Exit from the Service:

The service will be discontinued if:

• the service user no longer lives in a private residential care homes for persons with disabilities in Yuen Long District or Tuen Mun District

• the service user has stayed in hospital for more than three months and there is no date for his/her discharge.

• the service user or his/her family member applies for termination of the service

Saturday                9:00 am – 4:00 pm

(Closed on Sundays and public holidays)
